How to submit the completed questionnaire?
For individual registered healthcare personnel, please return the completed questionnaire to the Health Manpower Section by mail at the following address:

Alternatively, you can choose to submit the completed questionnaire through e-mail. In so doing, you are advised to make use of the e-Cert Certificate issued by the Hongkong Post and send the completed questionnaire in encrypted format to us at hms@dh.gov.hk so as to protect the confidentiality of your personal data. Please refer to Q.14 – 18 of theQ & Afor details.
