Scheme Overview
(i) Target Participants and Scope of Service
The Phase 2 of Pilot Scheme on Dental Services for Civil Service Eligible Persons (“CSEPs”) in Shenzhen (“PDSSZ2”) will arrange about 3 000 CSEPs to receive designated dental services, including oral imaging (X-ray) check and dental scaling services, at a medical institution in Shenzhen. CSEPs participating in the PDSSZ2 must have reached 18 years of age. The Department of Health (“DH”) will first invite CSEPs who have registered by e-Registration Form from January 2024 onwards for queuing for government dental services but have yet to receive dental services to participate in the PDSSZ2.
Subject to availability of quotas, other CSEPs would also be invited.
Participation in PDSSZ2 is voluntary and will not affect CSEPs’ entitlement to DH’s dental services in future.

(iii) Invitation Arrangement
Starting from February 2026, DH will invite CSEPs in batches to complete the Expression of Interest (“EOI”) Form through SMS (#DH-CLINICS) and email (cims2_notification@dh.gov.hk). The invited persons have to complete and return the EOI Form to express their interest in participation within two weeks. Submission of reply after the deadline or no reply will be considered as not joining the PDSSZ2. They will continue to wait for the dental service appointments at the government dental clinics (“GDCs”), and join the local Pilot Scheme on Dental Services (Dental Scaling) (“local PDS”) (if invited).
As there is a quota limit for the PDSSZ2, DH may not be able to invite all CSEPs who have indicated interest to join.

(iv) Procedures for Making a Dental Services Appointment
Starting from 26 February 2026, DH will issue formal invitations for participation in batches through SMS and email to CSEPs who have indicated their willingness to participate in the PDSSZ2. The invited persons should make an appointment with the NFUFH by calling the customer service hotline (+852 5801 1515 / +86 4008 919191) within one month upon receipt of the invitation, and receive dental services within two months from the date of receiving the invitation.
The invited CSEPs should identify themselves as eligible persons when booking appointment with the NFUFH. If the invited persons do not identify themselves as CSEPs during the booking process, they may be required to bear the costs incurred from the dental services.
During the booking process, the invited CSEPs should provide their full name in English (as shown on the Hong Kong Identity Card or other identity document used for dental consultation registration), the prefix letter(s) and first 3 digits of Hong Kong Identity Card or PMI number (10 digits) as shown in DH’s invitation SMS/email (also applicable to other identity document holders), and contact mobile number.
The NFUFH will confirm the appointment date and time with the CSEPs by SMS after booking, and also call and send SMS to remind them 24 hours before the appointment. CSEPs should attend the appointment as scheduled.
(v) Completion Code
Once an appointment is made, DH will send an SMS/email containing a completion code to the CSEP on the following day. The completion code should be provided to the NFUFH after completion of dental services to facilitate the validation process. If CSEPs do not receive the completion code before the appointment, or if they have lost the completion code, please call DH enquiry number at 2892 1044 or send an email to pdsvcode@dh.gov.hk for follow-up.

(vi) Rescheduling or Cancelling Appointment
CSEPs who wish to reschedule or cancel an appointment should contact the NFUFH for arrangement before the scheduled appointment. Please note that the date of rescheduled appointment should be within two months upon receipt of the invitation from DH. DH will send another SMS/email containing the completion code to the CSEP concerned two calendar days before the rescheduled appointment.
(vii) Arrangement for No-show cases after Booking an Appointment
If CSEPs had made an appointment to receive services under the PDSSZ2 but did not receive such services eventually without notifying the NFUFH in advance to cancel the appointment, the concerned persons would no longer be eligible for the PDSSZ2 and would not be invited to join the local PDS. For CSEPs who have registered for new consultation, they should continue to wait for new dental consultation appointment at GDC; for old consultation cases, they should contact DH or relevant GDC to arrange a recall appointment afresh.

(viii) On Appointment Day of Dental Services
Upon arrival at the NFUFH, participants of the PDSSZ2 would need to fill in the “Patient Registration Form” at the main lobby on G/F first, and then proceed to the Dental Clinic on 4/F. They should present their Hong Kong Identity Card or other identity document, and sign the “Consent” part of the “Joint Consent and Acknowledgement (JC&A) Form” to confirm their agreement to participate in the PDSSZ2, allow the NFUFH to verify their identity and eligibility for receiving dental services and provide their relevant information to DH.
After receiving the dental services under the PDSSZ2, the CSEPs should sign the “Acknowledgment” part of the JC&A Form to acknowledge receipt of the dental services. Please also provide the completion code to the NFUFH. If they are unable to provide the completion code, they should sign a “Declaration Form”.
The Government will only pay for the dental services provided under the PDSSZ2, and will not be responsible for the costs of any other treatments.

(ix) Marketing Activity
Staff of the NFUFH should not engage in active marketing of their dental treatments or services outside the scope of the PDSSZ2. However, if the CSEPs who participate in the PDSSZ2 choose to enquire and receive other dental treatments or services from the NFUFH outside the scope of the PDSSZ2 out of their initiation, they should sign the “Consent Form for Self-Pay Services” and bear the related service fees and the responsibilities themselves.
(x) Follow-up after Attendance
After completing the dental services under the PDSSZ2, the NFUFH will provide participants with a medical report. The participated CSEPs can upload and deposit the relevant medical record (i.e. consultation report) into their eHealth personal accounts through the "Personal Folder" function of the eHealth App, so that DH can access and for cases in need, schedule follow-up appointments at GDC. In this regard, participants are advised to download the eHealth App and register an eHealth account before attending the PDSSZ2 appointment. For other participants who do not have follow-up needs, DH will also schedule appointments for them at GDCs and notify them through SMS about three months before the appointment.

The participants will, on the following day upon receipt of dental services, receive an SMS/email from DH inviting them to fill in a service opinion survey. The survey will not collect any personal information, and all opinions and information collected will only serve the purpose of analysing the effectiveness of the PDSSZ2.
