1. Healthy Teeth Collaboration
Objective
To provide free oral check-ups, dental treatments and oral health education for adults with intellectual disability and/or, autism spectrum disorder.

Target Beneficiaries
To apply for the services under Healthy Teeth Collaboration, persons with intellectual disability and /or with autism spectrum disorder (“the applicant”) must fulfil the following criteria:
- Aged 18 or above; and
- Holding valid documentary proof certifying the disability category as “intellectual disability”, “mental handicap”, or “autism spectrum disorder”

- Upon booking of first appointment, the application form can be obtained from the selected dental clinic at which the appointment is made or download here. Application form should preferably be filled in and signed before attending the first appointment at the selected dental clinic. Alternatively, the form can also be filled in and signed at the dental clinic during the first appointment.
- At the first appointment, the original copy of the following documents should be presented:

- Applicant’s Hong Kong Identity Card
- One of the following documents certifying the disability category as “intellectual disability” , “mental handicap”, or “autism spectrum disorder”

- Valid Registration Card for People with Disabilities issued by the Labour and Welfare Bureau; or
- Medical certificate or “Certificate of Disability Type for Healthy Teeth Collaboration (HTC01E)”(as attached to the application form) issued by a doctor registered in Hong Kong ; or

“Certificate of Disability Type for Healthy Teeth Collaboration (HTC01E)”(as attached to the application form) issued by the person-in-charge of a rehabilitation service unit under the designated types of rehabilitation services#
#Designated types refer to the institutions set out in the Social Welfare Department of the Government’s website at <https://www.swd.gov.hk/en/pubsvc/rehab/> consisting of : care and attention home for severely disabled persons; hostel for severely mentally handicapped persons; hostel for moderately mentally handicapped persons; supported hostel; hostel for severely physically handicapped Persons with mental handicap; integrated vocational training centre – residential service; day activity centre; sheltered workshop; integrated vocational rehabilitation services centre; integrated vocational training centre – day; district support centre for persons with disabilities and day care service for persons with severe disabilities;
- If the applicant is unable to attend the first appointment on his/her own and is accompanied by a legal guardian appointed by the Guardianship Board under the Mental Health Ordinance (Cap.136), the legal guardian should present the following original documents:

- Documentary proof of the legal guardianship; and
- Legal guardian’s identity proof
- If the applicant does not have a legal guardian, applicant’s parent, grandparent, brother, sister, spouse or rehabilitation service unit may submit application and attend the first appointment with the applicant. The accompanying person should present original of his/her identity proof and documentary proof of his/her relationship with the applicant.
(Note : the participating non-governmental organisation dental clinic will make copy of the relevant documents and such copies will be submitted to the Department of Health for processing of fees reimbursement.)
Scope of Services
Non-governmental organisation dental clinics will provide:
- Oral check-ups
- Preventive and curative dental treatments
- Education for caregivers on daily oral care
Scope of dental services include radiographs, preventive treatment, treatment of gum diseases, fillings, extractions (including wisdom teeth), and/or pulp treatment.
If dental treatments cannot be provided under behavioural management at clinic, arrangements will be made for applicant to receive dental treatments under conscious sedation, deep sedation or general anaesthesia at a designated collaborating private hospital or day procedure centre.

Other information requiring attention
- Apply with one non-governmental organisation dental clinic only.
- Application can be made for escort subsidy and transport subsidy under this scheme. The subsidy limits and details can be inquired about at the participating non-governmental organization dental clinics. The escort subsidy and transport subsidy provided by this scheme are supplementary in nature and may not fully cover the actual costs incurred. Service users need to pay the difference so caused.
- Provide accurate and true information to the non-governmental organisation dental clinic.
- Arrange the person with intellectual disability and/or autism spectrum disorder to attend appointment at the non-governmental organisation dental clinic or designated collaborating private hospital as scheduled.
- If the scheduled appointment cannot be attended, the dental clinic should be notified as soon as possible to make alternative arrangements.
This Programme extends from 16 July 2024 until 31 March 2027 (both days inclusive).
2. Outreach Dental Care Programme for the Elderly (ODCP)
Objective
Users receiving long-term care services are generally frail and may be unable to take care of themselves and unable to receive conventional dental care services. Oral health of these users may get deteriorated as a result. Oral care is an integral part of overall personal and nursing care services for elders receiving long term care services.
The Government has launched the Outreach Dental Care Programme for the Elderly. Professional oral checkup and oral health care for elders residing in residential care homes, nursing homes or using service of day care centres and training is provided for their caregivers to promote prevention and early treatment, and enhance oral hygiene of the frail elders.

Target Beneficiaries
Person who holds a valid Hong Kong Identity Card, and residing in Residential Care Homes, Nursing Homes or Care and Attention Homes for the Aged Blind licensed by the Social Welfare Department, or aged 60 or above and using services of Day Care Centres/Units for the Elderly listed by the Social Welfare Department.
Scope of Services
Non-government organisations appointed by the Government will deploy outreach teams to provide the following free services:
- On-site oral checkup for the elders
- On-site oral care training and oral health education activities such as seminars/talks, etc for elders, their family members and caregivers
- If the elder is considered suitable for further curative dental treatment by dentist of the outreach team, treatment will be provided either on site or the elder will be transferred to a dental clinic for further dental treatment.

3. Special Oral Care Service
Objective:
To improve the oral health of children with intellectual disability, a Special Oral Care Service (SOCS) is set up by the Department of Health (DH) in collaboration with Hospital Authority at Hong Kong Children's Hospital for pre-school children of age under 6 with intellectual disability for early prevention and treatment of common oral diseases.

Targeted Beneficiaries:
- Children under age 6 with intellectual disability who are receiving service at Special Child Care Centres under Social Welfare Department will be provided with free on-site oral examination and oral health education. If necessary, children can be referred to SOCS at Hong Kong Children's Hospital for follow up treatment.
- Children under age 6 with intellectual disability can be referred to SOCS at Hong Kong Children's Hospital through the dental clinics of local public hospitals for timely dental treatment and oral health education.
- In-patients of Hong Kong Children's Hospital who are participants of the School Dental Care Service of DH but unable to attend consultation at designated school dental clinics due to hospitalisation can receive dental service at the Hospital while hospitalised.
Scope of Services :
The paediatric dental team of DH will provide:
- Oral examination
- Oral health education to parents/caregivers to promote daily oral care and encourage good dental habits
- Preventive treatments such as application of fluoride
- Basic dental treatments such as fillings, extractions
- Dental treatment under sedation or general anaesthesia

4. Primary Dental Co-care Pilot Scheme for Adolescents (PDCC)
Objective:
PDCC guides adolescents to foster long-term relationship with private dentists or dentists of NGOs by subsidising part of the cost of dental check-up, so as to promote their life-long habit of regular dental check-ups for prevention of dental diseases. Each eligible person can receive the subsidised services once every calendar year from a participating dentist of his or her choice who have enrolled in the programme. The Scheme Participant is required to pay a co-payment fee.

Target Beneficiaries:
- Adolescent aged 13 to 17 (or will be aged 13 in the calendar year the application to enrol is made. A person will cease to be eligible once he reaches the age of 18);
- Holds a valid Hong Kong Identity Card; and,
- Has joined the Electronic Health System (eHealth).
Scope of Services
The scope of subsidised services provided by Private Dentists includes the following items:
- Dental check-ups
- Individualised oral health risk assessment
- Dental scaling;
- Personal self-care advice on oral care and personal lifestyle for maintenance of good oral health;
- Fluoride application as a risk-based follow-up procedure; and
- Check-up report and explanation of the Scheme Participant’s oral health risk and oral condition.

5.Community Dental Support Programme (CDSP)
Objective
CDSP aims to promote early identification and timely intervention of dental diseases among the underprivileged groups with financial difficulties through provision of subsidised services.
Eligible persons can choose a dental clinic operated by participating non-governmental organisations (NGOs) either by approaching the clinic him/herself or through arrangement by a Registered Social Worker to receive the subsidised services under CDSP. Participants are required to pay an administration fee.
Target Beneficiaries
- Holds a valid Hong Kong Identity Card or is a holder of a valid Certificate of Exemption; and
- Be current beneficiary/ recipient of:
- Old Age Living Allowance (OALA) administered by Social Welfare Department (SWD); or
- Community Care Service Voucher Scheme for the Elderly (CCSV) administered by SWD with Co-payment Category II; or
- Integrated Home Care Services (Frail Case)”, “Enhanced Home and Community Care Services” and “Home Support Services” subvented by SWD while paying Level 1 or 2 fee charged / with Co-payment Category I or II ; or
- Medical Fee Waiver put in place by the Hospital Authority; and
- Has joined Electronic Health System (eHealth)
Scope of Services
After assessment by the attending registered dentist from the NGOs appointed by the Government, the following services will be provided to participant within 180 days if considered suitable:
- Oral health assessment
- Medication for dental pain relief (if necessary)
- X-ray examination
- Dental filling or extraction as treatment for one tooth (as one “Teeth Filling/Extraction Quota”), with a maximum of three Teeth Filling/Extraction Quotas
