| Notes |
| (1) |
Request
forms should be properly filled in and signed.
Full particulars of the examinations required,
the nature of specimen submitted and brief
clinical notes are required. These requests
should be addressed to the Consultant Medical
Microbiologists / Consultant Pathologists,
Department of Health. Unless otherwise specified
or agreed by the Consultant Medical Microbiologists
/ Consultant Pathologists, all charges specified
below are payable in advance or on demand. |

The
Consultant Medical Microbiologists / Consultant
Pathologists may refuse any sample. |
| (3) |
Samples
submitted for examination will remain the
property of the Government and may be disposed
of in any way considered suitable by the Consultant
Medical Microbiologists / Consultant Pathologists. |
| (4) |
The
Government will not recognize any claims arising
from the results of any examination. |
| (5) |
The
cost for other special tests not listed below
will be determined by the Consultant Medical
Microbiologists / Consultant Pathologists
according to the actual cost of the test. |

Specimens
for laboratory testing must be referred by
registered medical practitioners. |

tests are accepted, i.e. the free test offered
is for confirmation and not a preliminary
screening test, and |

(ii)
All referrals have to be channelled through
private registered medical practitioners.

Examinations
for poliomyelitis, smallpox, rabies, Japanese
B encephalitis, Dengue and Hantaan viruses
are free of charge. |

The above charges
include the issue of one copy of the report
of examination.
